Aging and Disability Resource Center Image

The Aging & Disability Resource Center (ADRC) empowers older adults, people with disabilities, and caregivers to make informed choices and to streamline access to long-term support. The Ventura County ADRC provides a coordinated point of entry to ensure the populations we serve have access to the resources and support needed to remain independent in the community. The ADRC is comprised of a partnership between the Ventura County Area Agency on Aging (VCAAA) and the Independent Living Resource Center (ILRC).

The Care Transitions Program is a short-term (30-day) program to assist discharging hospital and skilled nursing patients with learning self-management skills as they transition back home.
